  Description

  The goal of the P2R program is to provide continuity of care to eligible inmates six months prior to their release and continue for three years post-release from incarceration by providing comprehensive case management and access to an innovative cloud-based Pathways platform to track and streamline each inmate's unique Pathway based on their desired goals.

  Voluntary basis to inmates who are assigned to General Population at SBCC, within 6 months of their release date or within their parole eligibility date, and have a pending home plan to the Tidewater area
